LOST/ DAMAGED PASSPORT

Documents to be uploaded with the application:

1. Upload an ICAO compliant photograph (mandatory)/ signature, Or share soft copy of ICAO compliant photograph. For more details on ICAO compliant photograph please refer guidelines.
2. Self attested copy of Passport (first and last page)
3. Self attested copy of Valid Visa or ACR card
4. Police Report on letter head (For Lost Passport)
5. Sworn Affidavit for LOSS/DAMAGE (from the applicant) before the Consular Officer (Annexure "F")
6. Affidavit stating that the applicant has not obtained any other nationality (Affidavit).

*Any other document decided by Passport Issuing authority.

Please note: Application forms without uploaded photo, signature and requisite documents will not be accepted. Do not forget to bring the originals of the uploaded documents for verification at the time of submission of the application.

Important Note:
Processing time required would be 15 to 30 working days based on the receipt of Police verification.
