FRESH PASSPORT FOR A NEWLY BORN CHILD

 

For a child born in the Philippines, Federated States of Micronesia and Palau to be registered as an Indian citizen, the application must be made within one (1) year of the child’s birth on the prescribed form. For registration visit   https://indiancitizenshiponline.nic.in/initial?formcode=09.

 

For registration of birth of a child beyond one year, an Affidavit needs to be submitted by the parents stating the reason why the child was not registered within one year of birth and that the child has not acquired citizenship of any foreign country. If one of the parents is a foreign national, parents will produce a letter from the Department of Foreign Affairs confirming that the child has not acquired citizenship of that country. Duly filled registration form must be submitted at the time of submission of the passport application at the Consular wing of Embassy


 Documents to be uploaded with the application: 

a) Upload an ICAO compliant photograph (mandatory)/ signature, Or share soft copy of ICAO compliant photograph. For more details on ICAO compliant photograph please refer guidelines.

(In passport application of minors, either father or mother can sign on behalf of the child)

b) Self attested copy of relevant (first and last page of passport) of both parents.

c) Self attested copy of Valid Visa or ACR card both parents. 

d) Self attested copy of birth certificate authenticated by Philippine Statistics Authority (PSA) and apostilled by the Department of Foreign Affairs (DFA) of the Philippines. 

 

Please note: Application forms without uploaded photo, signature and requisite documents will not be accepted. Do not forget to bring the originals of the uploaded documents for verification at the time of submission of the application.

 

Other Documentary requirements:

· Declaration of Parent/Guardian for Minor Passports (Annexure "D")

· If Passport applied by one parent when other parent not giving consent, then application must carry a declaration affirming the particulars furnished in the application about the minor as per “Annexure C.
