LIFE CERTIFICATE

An individual who needs a Life Certificate, for the purpose of drawing pension in India may apply for this service (or if an individual is required to produce Life Certificate before authorities in India for any other purpose). For this service, applicants have to apply in person only. 

Documents required :  

a) Duly accomplished form (Download Form here)  

b) 2 pieces 2x2 size photos  

c) Original Passport with photocopy  

d) Original ACR card with photocopy
