EMERGENCY CERTIFICATE
An Emergency Certificate (EC) is a one-way travel document which authorizes an Indian national to enter India and to whom a new passport cannot be issued immediately. This may be the case of lost/stolen/damaged/expired Indian passport.
Documents to be uploaded:
a) Self attested copy of passport (first and last page)
b) Self attested copy of Valid Visa or ACR card
c) 3 passport size photographs
d) Philippine National Bureau of Investigation (NBI) clearance (in original)
e) In case EC required against Lost/Damage Passport:-
· Sworn Affidavit for LOSS/DAMAGE (from the applicant) before the Consular Officer (Annexure "F")
· Affidavit stating that the applicant has not obtained any other nationality (Affidavit).
· Police report for lost passport
Please note: Application forms without uploaded photo, signature and requisite documents will not be accepted. Do not forget to bring the originals of the uploaded documents for verification at the time of submission of the application.
